Authorization to Administer Medication

Please complete this form if your child has medicine that needs to be administered while at AIDA.

All medications/bottles should be placed in a plastic bag clearly marked with your child's name, time to administer, and given to a team member. Students should not keep medications in lunchboxes, etc.
